The Telehealth Nurse is a member of the Home Health and Hospice team and is responsible for the daily management of patients receiving telehealth services and for the collection, analysis, trending and reporting of telehealth data. The Telehealth Nurse is also responsible for assuring the functioning of associated telehealth equipment, software and technical integration between documentation systems. Supervisory Responsibilities: This position has no direct supervisory responsibilities. Essential Job Functions/Responsibilities: Works closely with Complex Care Coordinator, QAPI Analyst, clinical staff and management to determine patients appropriate for remote patient monitoring and/or virtual visits. Assesses patients’ ability to participate in telehealth programs and facilitates all consenting documentation. Assesses patient care needs and prepares individualized patient centered telehealth care plans for remote patient monitoring and virtual visits. Installs telehealth equipment in patients’ homes following agency visit guidelines and educates patients/caregivers on use of telehealth equipment and best practices to ensure timely, accurate communication with health care professionals and desired health care benefits. Monitors remote telehealth data daily and follows up with patient/caregiver and their care team to coordinate modification of care plan when indicated. Delivers virtual telehealth nursing visits in accordance with the Vermont Nurse Practice Act and organization policy and protocols for specific disease management. Empowers patients to be active in their healthcare and promotes patient independence with chronic disease management. Documents all telehealth activity accurately and timely using secure software and in accordance with HIPAA guidelines. Facilitates telehealth equipment removal, tracking and packaging of malfunctioning or expired equipment. Ensures all telehealth equipment is accounted for and in good working condition. Works with IS Support to ensure all software interfaces are functioning appropriately. Serves as an agency champion for staff and community partners through identification of patients appropriate for telehealth services and demonstrates the value and successes of individual patients. Maintains professional and technical knowledge by attending educational workshops and reviewing professional publications. Assists with patient visits as needed. Assists with clinical and/or administrative call, weekend and holiday responsibilities on a rotational basis.
