About The Position

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness is an independent, non-profit healthcare provider licensed in 11 counties in northwest Illinois. We have a state-of-the-art facility and the first free standing in-patient Hospice Home in Illinois. We have a 35 year history for providing compassionate professional care to terminally ill patients and their families. We are state licensed and Medicare/Medicaid certified. We are currently seeking full-time RN’s to be Team Support Nurse to care for our patients residing in their private home, nursing home or long term care facility. Our Team Support Nurse will assist our Field Case Managers within a specific territory. This position will assist with the caseload of patients for that territory and provide care and visits according to the plan of care. The Team Support Nurse will assist with daily visits, prn visits and/or death visits as needed. They are responsible for providing loving care for patients in their activities of daily living. These positions work Mon-Fri and must have the availability to do minimum of 20 hours of on-call per month which includes a holiday rotation. Illinois Registered Nurse with a minimum of one year experience in either home health or nursing home/long term care. Previous Hospice experience helpful.
