About The Position

The Team Leader, Mobile Crisis Response is a part of the management structure of the program. The Team Leader is responsible for the direct supervision of the team in order to organize, direct, and develop quality service delivery, crisis assessment, and referral services to members of the community in need of crisis care. The Team Leader functions as the senior clinician or “player coach” of the team. This position reports to the Crisis Program Director. ESSENTIAL DUTIES & RESPONSIBILITIES Assumes the leadership role within the program’s assigned team and oversees the activities of crisis workers and engagement specialists performing Mobile Crisis Response (MCR) and assessment of members in the community to determine needs and make recommendations for treatment. Works collaboratively community stakeholders and develops relationships with providers and partners in the crisis care continuum. Assists the team in answering Thresholds crisis line and assessing caller to determine needs and level of intervention necessary. At implementation of the Statewide 988 crisis line, this position will assist the team in fielding these referrals and making determinations on the appropriate level of response. Models person‐centered, recovery‐oriented care that is rooted in evidence‐based practices aimed at assessing crisis and determining appropriate safety planning. Meets required targets and completes all documentation within assigned time frames. Monitors and coaches staff on meeting and documentation requirements. Monitors and tracks member data, call flow, and insurance eligibility daily and collates this into monthly reports for distribution to Thresholds leadership and funders. Represents Thresholds in monthly ED and behavioral health meetings to address ED operations, patient flow, monitor effectiveness of crisis services, and identify barriers and improve systems. Reviews all required documentation. Maintains the quality, validity and confidentiality of individual case records according to CARF, Medicare, Medicaid standards and agency traditions. Assures that all member files and documentation are completed accurately, on time and is organized in a systematic manner. Monitors employee compliance with required training. Assists with special tasks as assigned, including but not limited to: representing the agency at meetings, conferences, supervising students/interns, orienting and training new staff. Conducts annual evaluations in a timely manner for all team staff. Develops specific goals and objectives, and provide the method and opportunity for each team staff to succeed in their professional development. Participates in the hiring sequence of the agency, and make recommendations on the hiring or termination of team staff. Follows agency standards regarding clinical supervision. Assists Program Director with the development and coaching of staff, to ensure they consistently meet agency standards. Responsible for maintaining 24/7 coverage of the MCRT including the management of a substitute staff pool. Conduct MCR on an as needed basis when coverage is necessary for crisis workers. Required to be on‐call for assigned shift to assist in maintaining coverage of the MCRT and to provide clinical support. Must be able to remain in a stationary position for prolonged periods of time and occasionally traverse long distances and ascend/descend multiple flights of stairs. Shift: Sunday - Thursday 2:45pm-11:15pm MARGINAL DUTIES & RESPONSIBILITIES Works flexible schedule including work on weekends or evenings as required.

Requirements

  • Master’s degree in social work, counseling, psychology or closely related field is required.
  • LSW or LPC is required.
  • Two (2) years clinical experience.
  • Ability to synthesize and summarize information and make judgments regarding member care.
  • Computer proficiency, including the ability to use email, navigate and enter notes into electronic medical records and working knowledge of pertinent software.
  • Strong interpersonal skills and knowledge of group dynamics required.
  • May be required to provide transportation for members of the program using a Thresholds provided vehicle or personal vehicle.
  • Must have a valid driver's license, be at least 23 years of age, and must meet all insurance carrier’s requirements.
  • Must obtain and maintain $100,000/$300,000/$100,000 bodily injury and liability.
  • Subject to validation every six months.
  • Must be able to remain in a stationary position for prolonged periods of time and occasionally traverse long distances and ascend/descend multiple flights of stairs.

Nice To Haves

  • LCSW or LCPC is preferred.
