The Jewish Board’s Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resiliency of our communities. PROGRAM DESCRIPTION The ACT program provides an integrated set of evidenced based treatment, rehabilitation, case-management and support services to individuals residing in Shelters and/or living in supportive housing programs or their own residences in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care. POSITION OVERVIEW: The Team Director takes lead as a member of the ACT Team, which promotes a team based approach in its work with adults with behavioral health disorders living in homeless shelters or in the community. S/he is responsible for ensuring clinical services are provided using evidence based approaches and best practice, and managing day to day operations to ensure the program runs smoothly and with minimal disruptions to client services. The Team Director provides on-going training, supervision, and guidance to the clinical and office staff.
