Requirements

• Arizona Elementary Certificate with Middle Grades Math OR Math Approved Area AND Middle Grades General Science OR General Science Approved Area OR Arizona Middle Grades Certificate with Middle Grades Math OR Math Approved Area AND Middle Grades General Science OR General Science Approved Area OR Arizona Secondary Certificate with Middle Grades Math OR Math Approved Area AND Middle Grades General Science Approved Area OR Arizona Subject Matter Expert Certificate 6-12 with Math Approved Area AND Arizona Subject Matter Expert Certificate 6-12 with General Science Approved Area
• Current Arizona IVP Fingerprint Clearance Card
• Proof of immunity to rubeola (measles) and rubella (German measles), or proof of MMR immunization.
