About The Position

This position is responsible for providing case management, skills training, and psychosocial rehabilitation services to a caseload of adult mental health parole and/or probation customers, as outlined in their treatment plans. These specialized services are contracted by the Texas Correctional Office on Offenders with Medical & Mental Impairments (TCOOMMI). Key duties include conducting assessments, developing treatment plans, and coordinating services with parolees, their families, and parole officers. The role involves implementing skills training to address deficits in symptom management, medication training, accessing community resources, and community integration. The Case Manager will also perform crisis assessments and facilitate access to emergency psychiatric treatment when needed. Additionally, this position provides continuity of care (COC), transitional case management (TCM), and/or intensive case management (ICM) services to individuals on probation and parolees released from state prison, ISF, and SAFP facilities. The primary goal is to maintain consumers' mental stability to reduce recidivism to correctional facilities, transition them into lower levels of care, and support their reintegration into the community. The successful candidate must be able to work independently while maintaining professionalism with clients and community stakeholders.

Requirements

  • Requires a Bachelors Degree in Psychology or a Human Services Field from an accredited college or University
  • Must be eligible to be certified as a Qualified Mental Health Professional
  • Must possess a valid Texas Drivers License
  • Be an insurable driver
  • Maintain an acceptable driving record/insurability while employed

Nice To Haves

  • Prefer previous mental health work experience with adult mental health consumers

Responsibilities

  • Provide case management, skills training and psychosocial rehab to a caseload of adult mental health parole and/or probation customers as prescribed in the consumers’ treatment plans of care
  • Conduct assessment, treatment planning and coordination of services with the parolee, family and parole officer
  • Implement skills training services to reduce consumers’ skills deficits in the area of symptom management, medication training, accessing community resources and community integration
  • Complete crisis assessments and help assigned consumers access emergency psychiatric treatment as needed
  • Provide continuity of care (COC) services, transitional case management (TCM) services, and/or intensive case management (ICM) services to people on probation and parolees released from the state prison system, ISF facilities and SAFP facilities
  • Provide case management, skills training, psychosocial rehab and crisis services to the caseload with the goal of maintaining their mental stability to reduce recidivism back to state prison and other corrections facilities
  • Work to transition the consumers into lower levels of care and to reintegrate the individual back into the community
  • Work independently, while maintaining professionalism with clients and community stakeholders
