We are seeking a Transitional Age Youth (TAY) Specialist to support youth ages 13–26 in building the skills and confidence needed for independent living. Under the direction of the Direct Service Manager, you will provide one-on-one guidance and mentorship—particularly to youth involved in foster care, DDD (Developmental Disabilities Division), and Title 19 programs—while helping them develop essential life skills such as budgeting, employment readiness, housing navigation, and self-advocacy. You will conduct assessments, create individualized transition plans with measurable goals, facilitate group workshops, and collaborate with community partners and schools to ensure comprehensive support. Maintaining accurate documentation, completing required training, monitoring program effectiveness, and incorporating best practices into service delivery are key responsibilities.

Requirements

  • Must be at least 21 years old
  • Possess a high school diploma or equivalent
  • AZ Driver's License
  • Level 1 Fingerprint Clearance Card
  • At least one year of experience in behavioral health or a related field
  • Strong written and verbal communication skills
  • Organizational abilities
  • Proficiency with Microsoft products
  • Ability to maintain confidentiality
  • Ability to work in both indoor and outdoor environments, including extreme weather conditions
  • Physical requirements include prolonged sitting, frequent standing and walking, bending and stooping, lifting up to 50 pounds, and occasional stair climbing
