The Transitional Age Youth (TAY) Specialist supports youth ages 13–26 in building the skills and confidence needed for independent living. Under the direction of the Direct Service Manager, this role provides one-on-one guidance and mentorship, particularly to youth involved in foster care, DDD (Developmental Disabilities Division), and Title 19 programs, while helping them develop essential life skills such as budgeting, employment readiness, housing navigation, and self-advocacy. Key responsibilities include conducting assessments, creating individualized transition plans with measurable goals, facilitating group workshops, collaborating with community partners and schools to ensure comprehensive support, maintaining accurate documentation, completing required training, monitoring program effectiveness, and incorporating best practices into service delivery.

Requirements

  • At least 21 years old
  • Possess a high school diploma or equivalent
  • AZ Driver's License
  • Level 1 Fingerprint Clearance Card
  • At least one year of experience in behavioral health or a related field
  • Strong written and verbal communication skills
  • Organizational abilities
  • Proficiency with Microsoft products
  • Ability to maintain confidentiality

Nice To Haves

  • Peer Support Certification, or candidates who meet the qualifications to enroll in the certification course
  • Bilingual skills

Responsibilities
