The Targeted Case Manager (TCM) will provide case management services to individuals in their assigned program who have severe mental illness (SMI), substance use (SUD), and/or serious emotional disturbance (SED). The position involves resource acquisition, linking to services, coordinating care for physical and mental health needs, and monitoring the changing needs of participants. Duties include assessing client and family strengths and needs, linking them to appropriate community resources, assisting with advocacy efforts with other community services, and maintaining appropriate contact with clients and/or families, including a minimum of 4 contacts a month with at least 2 being face-to-face. The TCM will collaborate with Lifeskills staff and external providers to assist in the client’s recovery, assure client choice and access to community resources, and provide services in both clinic and community settings (e.g., homes, schools, courts, DCBS). This role also involves assisting individuals in engaging in activities that provide opportunities for developing meaningful roles in their community, using a personal vehicle to transport individuals, conducting oneself morally and ethically as a positive role model, providing billable targeted case management services to 23-25 clients per month, and timely documentation of services. The TCM may also fill in for other staff during absences and attend scheduled individual and group supervision.
