About The Position

If you are passionate about supporting adults in navigating services, accessing resources, and achieving greater stability and independence, COMHAR invites you to join our team as a Targeted Case Manager. In this role, you will help empower individuals to meet their goals and make a meaningful difference every day. COMHAR is seeking Targeted Case Managers to join our team. In this role, you will work directly with adults, including young adults (18–25) and members of the Hispanic community through our bicultural/bilingual team, to assess needs, develop service plans, and coordinate community-based supports. You will meet clients in their homes, hospitals, or other community settings, providing hands-on guidance and advocacy to help them navigate services and achieve their goals.

Requirements

  • High school diploma or GED with 12 college credits in behavioral health/human services required.
  • 2–4 years of CMI experience.
  • Current Driver's License; satisfactory driving record; eligible to operate program vehicles.

Nice To Haves

  • Associate's degree or Bachelor's degree (must be in Social Work, Psychology, or other Human Service field) preferred.
  • Bilingual proficiency in English and Spanish, with strong reading and writing skills is preferred but not required.

Responsibilities

  • Accomplishes clients' goals and objectives by assessing needs and elaborating service plan; monitoring the client's mental and physical health, and linkage to community resources in order for the client to gain the needed tools to navigate the system and promote independence.
  • Maintains clients' records by reviewing case notes; and service plan reviews.
  • Submitting documentation for all services provided.
  • Participating in daily morning meetings and individual supervision.
  • Supporting individuals in crisis management and verbal de-escalation.
  • Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation.
  • Monitors cases by verifying clients' attendance to providers; observing and evaluating treatments and responses; advocating for needed services in the community; obtaining additional resources
  • Maintaining productivity requirements of the program.
  • Meeting individual where they are and providing services in the communities they live in.
