About The Position

The YMCA of Orange County is hiring Tailored Day Service Facilitators to support adults with disabilities who are pursuing higher education. These positions will be from Monday to Friday and hours per will will range from 15 to 20 hours. The Facilitator will support the Y Inclusion program in order to facilitate the inclusion support for the community.

Requirements

High school diploma or equivalent
12 college units with 6 units in Child Development, Psychology, Sociology or Education required.

Nice To Haves

1-year experience working with special needs/developmentally disabled population preferred.

Responsibilities

Provide Community Integration Services to participants referred from the Regional Center of Orange County
These duties include assessment, discovery, goal writing, and implementation of the Individualized Service Plan with the participant
Case Management Services as needed
Coordinate Services of own cases with Regional Center
Provide Person-centered Care
Maintain all Health and Safety Protocols
Drive the Van and the clients in the van
Maintain van safety as described in the P&P’s
Collect data and write Progress Reports
Meet with community businesses, services and educational organizations as needed
