About The Position

We're currently looking for a compassionate and committed Tailored Care Manager to become part of our supportive, multidisciplinary Tailored Care Management (TCM) team in Johnston County . Easterseals PORT Health’s Tailored Care Management (TCM) team is committed to delivering whole-person, integrated care that truly makes a difference. As a Tailored Care Manager , you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands-on role with the opportunity to travel throughout Johnston and surrounding counties , helping close gaps in care and improving lives in the community every day.
Requirements
- Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area.
- Eligible for Qualified Mental Health Professional (QMHP) credentialing
- Minimum 2 years of experience with mental health and/or substance use populations
- Proficiency in Microsoft Office and Electronic Health Records (EHR)
- Valid driver’s license, current auto insurance, and a clean driving record
- Strong communication skills and the ability to work independently
- Flexibility to provide services in various community settings and participate in rotating after-hours support.

- Master’s level degree preferred.

Benefits
- Competitive salary ranging from $52,000 to $54,000 annually, based on your experience and qualifications.
- Full-time role offers a primarily Monday–Friday schedule from 9 AM to 5 PM , with rotating on-call responsibilities.
