About The Position

We're currently looking for a compassionate and committed Tailored Care Manager to become part of our supportive, multidisciplinary Tailored Care Management (TCM) team covering Bertie and Chowan counties. As a Tailored Care Manager , you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands-on role with the opportunity to travel throughout Bertie, Chowan counties. and surrounding counties , helping close gaps in care and improving lives in the community every day.

Requirements

  • Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area.
  • Eligible for Qualified Mental Health Professional (QMHP) credentialing
  • Minimum 2 years of experience with mental health and/or substance use populations
  • Proficiency in Microsoft Office and Electronic Health Records (EHR)
  • Valid driver’s license, current auto insurance, and a clean driving record
  • Strong communication skills and the ability to work independently

Nice To Haves

  • Master’s level degree preferred.
