About The Position

Description of Position : Primary responsibilities include performing assessments and ongoing counseling for both families and children as well as providing case management to link people to services needed for success. Will utilize evidence-based practices focusing on juvenile risk factors to improve mental health and to reduce the likelihood of delinquent behavior, including truancy. Services will be strength based and family driven .

Requirements

  • Bachelor degree in psychology, social work or human development.
  • Possession of a valid South Dakota driver's license.
  • Must live within 60 miles of Watertown to allow for timely response to crisis evaluations
  • Cannot be in the Office of Inspector General (OIG) exclusionary database.

Responsibilities

  • Consult with schools and community agencies.
  • Accept referrals and ensure completion of intake paperwork.
  • Assess and assist families with needs across the life domain and ensure they are informed of service options and community resources.
  • Complete intakes and conducts individual and family therapy as indicated by needs assessment.
  • Complete all documentation including assessment, treatment plans, progress notes & outcome measurement tools.
  • Track students who are deemed truant by the school district & host team meetings to establish services necessary to restore the student to daily attendance & success.
  • Conduct an assessment of students identified as truant and make referrals or provide in-home individual and family therapy as needed.
  • Submit monthly SOC billing spreadsheet to the division in addition to regular documentation.
  • Participate in in-service trainings and staff meetings.
  • Complete accurate, timely and comprehensive documentation of all clinical services.
  • Make recommended documentation or clinical changes outlined by supervisor in quality assurance reviews.
  • Actively provide input in improving the overall quality of services provided both personally and by other departments.
  • Provide crisis intervention service to clients in the on call rotation and document the interaction in electronic health record.
  • Complete all required documentation related to client service delivery by month end.
  • Complete written evaluation within 30 days of client intake.
  • Complete treatment plan within 30 days of client intake.
  • Carry a caseload of no more than 20 families in SOC.
  • Complete treatment plan reviews and outcome measures per ARSD rule.
  • Terminate clients in EHR within ARSD rule timeline.
  • Seek out consultation in a timely manner with supervisor(s) or clinical team when in doubt.
  • Keep supervisor(s) informed of atypical clinical situations as well as informs crisis rotation team of potential crisis situations with client case load.
  • Maintain discretion and confidentiality as required in all communication with others.
  • Provide professional & educational presentations to the community as needed.
  • Demonstrate a positive image of the Human Service Agency by applying effective human relation skills with clients, visitors and other employees.
  • Stay informed of the latest research, best practices and continuing education opportunities in the mental health field.
  • Seek continuing education pertinent to position on an annual basis.
  • Abide by the philosophy, mission, and ethical standards of the Human Service Agency.
