About The Position

We’re looking for a Supervisor of Care Coordination, someone who’s ready to grow with our company. In this position, you will supervise front line clinical and non-clinical staff by directing the activities for the team to meet regulatory requirements, internal best practice guidelines, and ensure positive member outcomes. Ensure resources are utilized efficiently, in accordance with program objectives, key performance indicators, and in ways that consistently promote Member quality and satisfaction. Ensure Member complaints and/or concerns are resolved efficiently through appropriate communication and intervention. Handle Member concerns and issues that require a high degree of appropriate knowledge, creativity, research and communication with internal and external resources.

Requirements

  • Bachelor’s degree and either a Nursing Diploma, an Associate Degree in Nursing, or an associate degree in a related field recognized by the Texas Board of Nursing for RN licensure are required
  • RN - Lic-Registered Nurses by the Texas Board or Nursing or Nurse Licensure Compact required
  • 1-year managed care experience required
  • 3 years' experience in population health, community public health, ambulatory care, or healthcare administration required

Responsibilities

  • Support care management, population health, and/or behavioral health during daily operations.
  • Assess, develop, implement and monitor a comprehensive plan of care through an interdisciplinary team process in conjunction with the member/family in internal and external settings.
  • Perform all necessary communication and documentation functions.
  • Collaborate with community health education/outreach organizations and providers to provide for the overall health promotion of members.
  • Conduct provider and member education activities.
  • Prepare, monitor, and audit outcome data to assist in identifying improvement opportunities.
  • Ensure team members have the necessary tools and information to perform their job.
  • Collaborate with other departments to resolve member and/or provider issues.
  • Ensure Team provides case management, discharge and service planning review for members including assessment, planning, implementation and evaluation of educational, Long-Term Services & Supports (LTSS), and social service-related options.
  • Conduct initial and ongoing system training for Case Management staff.
  • Participate in quality initiatives and performance management.
  • Supervision of daily operations.
