About The Position

Join our team of Clinical Supports Services Team! As a member of the Clinical Supports Services Team, the Supports Coordinator Assistant is essential member of the team and provides recovery-based services and supports to adults who meet criteria for a serious mental illness (SMI),and/or Intellectual and/or developmental disability [I/DD] and with individuals living with a co-occurring substance use disorder through the implementation of Person-Centered Planning. The Supports Coordinator Assistant is responsible for supporting their designated Case Manager/Supports Coordinator(s) in assisting clients in designing and implementing strategies for obtaining services and supports, that are goal-oriented and individualized. Your office is surrounded by nature, located in Benzie and Manistee Counties, just miles from beaches and hiking trails. About the Role The Supports Coordinator Assistant works with adults living with Severe Mental Illness (SMI) and adults living with Intellectual/Developmental Disabilities (I/DD), and individuals living with co-occurring substance use disorder. Services include assessment, documentation, assisting in planning, linkage, advocacy, and coordination and monitoring to assist clients in gaining access to needed health services, financial assistance, housing, employment, education, social services, and other services and natural supports developed through the person-centered planning process. The Supports Coordinator Assistant’s services are based on the principles of recovery and self-directed services and are provided in a responsive, coordinated, effective and efficient manner.

Requirements

  • High School Diploma or GED

  • Preference will be given to applicants who are currently enrolled in or have completed an undergraduate Human Services program, with particular priority for those pursuing a Bachelor of Social Work (BSW).

Responsibilities

  • Participate in quality person centered planning for clients and ensure its implementation based on the principles of recovery and trauma informed care.
  • Ensure the implementation of self-directed services principles that incorporate strengths and abilities.
  • Support clients in increasing independence, remaining in their least restrictive environment, and improving their quality of life.
  • Provide direct face-to-face support and service coordination activities.
  • Meet clients and their identified support persons as appropriate at times and locations, including outreach in clients’ homes and in the community, chosen by the client, guardian or other important persons involved in the client’s care and recovery plan.
  • According to person/family centered plans, provide advocacy, linking, coordinating, and monitoring to assist with housing, education, transportation, employment, financial, social, psychiatric, and other clinical and community inclusive activities, including assistance with discharge planning when appropriate.
  • Provide coordination of care for physical health care and community resources, including coordinating care with Primary healthcare providers and specialist.
  • Monitor mental and physical health status, including monitoring medication use and management and communicates observations, concerns and recommendations to the Primary Case Manager.
  • Provide appropriate interventions designed to stabilize and improve functioning and assists the clients to experience a positive recovery process from a severe mental illness and/or co-occurring disorder.
  • Provide timely clinical documentation in the electronic medical record that includes amount, scope and duration of services provided in accordance with the person-centered plan.
  • Comply with agency policy, MDHHS, CARF, and Medicaid criteria for clinical documentation for all aspects of the medical record including, but not limited to, intake assessments, person-centered plans, person-centered plan reviews, releases of information, progress/activity notes, annual psycho-social assessments and discharge summaries.
  • As medically necessary, coordinate skills training and support in daily living skills and activities designed to increase independence and foster recovery through direct education, modeling, prompting and shaping behavior, and social contact.
  • Promote recovery and respects the choices and autonomy of clients while teaching and encouraging actions that will promote good mental and physical health.
  • Participate in team meetings and other staff meetings as requested to maintain proficiency and develop competency, and meet regularly with supervisor for consultation, supervision, and resource development.
  • Contribute to team effort by accomplishing related results as needed.
  • Perform duties and responsibilities in ways that promote goodwill, build positive relationships.
  • Preserve human dignity and contribute positively to the CWN service organization.
  • Actively participate in the mission and vision of CWN and practices using sensitivity to race, ethnicity and culture as well as assuring Trauma Informed Practices.
  • Comply with all policies and procedures of the agency.
