The Supportive Services Specialist is responsible for providing supportive services to residents living in HUD buildings. These services promote housing stability, improve access to health and community support, and strengthen residents’ ability to live independently through individualized assessment, service linkage, and skill-based coaching. The Supportive Services Specialist will provide voluntary, resident-centered services using a strengths-based, trauma-informed approach. Services will be offered to current and new tenants through outreach and engagement strategies. Once accepted a functional assessment along with Social Determinants of Health screening will be completed. Services will begin with an initial assessment and service plan, followed by ongoing check-ins based on resident needs and preference (e.g., scheduled office hours onsite, home visits when appropriate, and phone/text follow-up). The Supportive Services Specialist will coordinate closely with the HUD Coordinator and Maintenance to support tenancy.
