About The Position

The Supportive Housing Specialist provides intensive short- and long-term case management to individuals and families who reside in supportive housing. This role assesses client needs, develops individualized housing plans, and coordinates access to mental health, medical, social, educational, vocational, and other essential services. The position works closely with community agencies, property managers, and support networks to promote housing retention, independence, and overall well-being, while ensuring compliance with program requirements and maintaining accurate documentation. This position is made possible by an awarded grant and funded through June 2027. This position has potential to become full-time, if further funding becomes available.

Requirements

  • Associate degree in social work or related field and a minimum of one (1) year of related experience working with persons with disabilities, persons aged 65 or older, low-income households, homeless populations, substance use and/or behavioral health programs; OR an equivalent combination of education and experience.
  • Must have a valid driver's license, current car insurance, meet insurability standards, pass a background check, and have an internet-enabled device.
  • Proficiency in speaking and writing in the English language is required.
