About The Position

The Supportive Housing Case Manager provides on-site, trauma-informed case management services to residents living in Permanent Supportive Housing (PSH) units to promote long-term housing stability, wellness, and self-sufficiency. This position serves as the primary service coordinator for assigned participants, processes referrals from internal and external partners, and ensures compliance with funder, contract, and HMIS documentation and reporting requirements. This role may provide services across multiple PSH sites and works collaboratively with property management, clinical providers, and community partners to support successful housing outcomes.

Responsibilities

  • Provide on-site, participant-centered, trauma-informed case management services to residents living in Permanent Supportive Housing units
  • Serve as the primary point of contact for assigned residents to coordinate housing stability, health, behavioral health, and social service supports
  • Provide services across more than one housing site as assigned based on program needs
  • Process and manage referrals from external partners and coordinated entry systems
  • Conduct outreach, engagement, intake, assessment, and individualized service planning
  • Support participants with housing stability, benefits acquisition, and connection to community resources
  • Provide move-in support and ongoing wraparound services
  • Participate in case conferencing and cross-department collaboration with property management, clinical teams, and external partners
  • Partner closely with property management staff to promote lease compliance, prevent evictions, and support successful tenancies
  • Maintain a consistent on-site presence in alignment with program and contract requirements
  • Attend required meetings with funders, system partners, and community stakeholders
  • Ensure all participant services, enrollments, and documentation are entered into HMIS accurately and on time
  • Maintain compliance with HMIS data quality standards, audit requirements, and reporting deadlines
  • Maintain organized, accurate, and audit-ready participant files and records
  • Work independently while maintaining strong communication with supervisor and team
  • Adhere to Housing First, Harm Reduction, Low-Barrier, and Trauma-Informed Care principles.
  • Maintain professional boundaries and ethical standards.
  • Participate in required trainings and staff meetings.
  • Maintain flexibility in scheduling to meet participant and program needs across both sites.

  • Represent New Narrative professionally in cross-agency coordination.
