As a Supportive Housing Case Manager, you will be primarily responsible for providing ongoing, structured home- and community-based case management services to participants participating in the Supportive Housing program. The Housing Programs Department serves households in Hennepin County who are experiencing or at risk of homelessness. Our programs each provide tailored support to help households stabilize housing and achieve self-sufficiency. Position Responsibilities: Provide home and community-based case management services for participants to assist with finding and maintaining housing (i.e. assessment, goal planning, and regular in-home visits). Manage multiple sources of funds to assist participants and maintain accurate records. Partner with Housing Locator to assist participants in finding and maintaining housing through building relationships with landlords and provide landlord-tenant support and mediation along with then maintaining participant files and complete accurate and timely documentation including daily case notes, and data collection. Conduct monthly home visits with each assigned participant and offer regular and quality contact with participants to maintain trust, assist in crisis prevention, and assist in meeting individual needs to then develop individual goal plans with participants and provide supports necessary to achieve participant-identified goals. Support and effectively work with participants with diverse identities and experiences including young participants; participants with cultural backgrounds of historical and generational trauma; participants experiencing past or current domestic violence; participants struggling with mental, physical, developmental, and chemical health challenges; and participants with persistent low income. Communicate and coordinate services with other program staff. Attend and participate in team meetings. Provide flexible services during times that meet the needs of participants. Position consists of approximately 50% direct, primarily community-based, participant contact, and 50% indirect participant-related work (i.e. documentation, communicating with collateral partners, preparing for participant meetings, attending trainings and community events).
