About The Position

As a Supported Living Assistant Manager at Pathfinder, you play a hands-on leadership role in ensuring individuals receive safe, consistent, and person-centered supports in their homes. Working closely with the Supported Living Manager, you support Direct Support Professionals (DSPs), oversee daily residential operations, and help create environments where individuals can thrive physically, emotionally, and socially. As a Supported Living Assistant Manager at Pathfinder, You Will: Team Leadership & Supervision Supervise and support Direct Support Professionals (DSPs), reinforcing expectations and person-centered practices. Conduct regular team meetings to promote communication, accountability, and quality care. Serve as the first point of contact for DSPs regarding scheduling adjustments and coverage needs. Escalate staffing or operational concerns to the Supported Living Manager as needed. Participate in leadership on-call rotation to support continuity of care. Person-Centered Supports & Planning Collaborate with the Supported Living Manager to implement Person-Centered Individual Support Plans (PCISPs). Ensure supports align with individual preferences, goals, and outcomes. Participate in PCISP, quarterly, and interdisciplinary meetings as directed. Promote dignity, independence, and quality of life in daily supports. Compliance, Documentation & Quality Assurance Ensure daily operations meet safety, health, and quality standards. Maintain accurate case notes, billing records, and Medicaid Waiver documentation. Monitor and enforce staff training compliance. Ensure proper medication administration, documentation, and timely refills. Uphold confidentiality, ethical standards, and regulatory compliance. Operations, Scheduling & Coverage Oversee daily operations across three designated residential sites supporting 9–12 individuals. Coordinate medical appointments, day program participation, and transportation logistics. Ensure coverage for open shifts by adjusting schedules and coordinating staff. Identify and report major home maintenance needs. Manage residents’ personal finances in alignment with agency financial protocols. Communication & Family Engagement Act as a liaison between DSPs, families, guardians, and the Supported Living Manager. Ensure timely communication of updates, concerns, and care needs. Support continuity of care through clear, consistent communication across teams. Requirements

  • At least 18 years of age.
  • Basic language, written, and computer skills necessary for communication and documentation.
  • Demonstrated ability to support people with disabilities.
  • Ability to work a flexible schedule if needed.
  • Crisis prevention, intervention, and teaching skills.
  • Valid driver’s license, insured vehicle, and safe driving record.
  • Ability to speak, hear, and read the English language.

Nice To Haves

  • Minimum of a high school diploma/GED.
  • Prefer 2 years experience in the field of Human Services.
