About The Position

The Supported Living Coordinator (SLC) is responsible for mobilizing and directing team members to ensure services are delivered as outlined in each CRI Person Centered Support Plan (PCSP). The SLC delegates responsibilities based on staff strengths while maintaining overall accountability for service provision in accordance with the Code of the State of Iowa, contractual requirements of funding authorities, and agency policies and procedures. This position represents the organization in various professional settings and ensures services are implemented effectively, ethically, and within established guidelines.

Requirements

  • Able to obtain and maintain certification in Medication Management, CPR, and Safety-Care within the first 6 months of hire.
  • Bachelor’s degree preferred or an equivalent combination of education and experience.
  • One year of experience working with persons with disabilities.
  • Strong communication skills and people skills
  • Ability to work independently and as part of a team
  • Ability to develop and implement CRI PSCPs in a non-biased, accurate, and client-centered manner.
  • Interact in an empathetic and professional manner with clients.
  • Always maintain confidentiality in all aspects of this position.
  • Ability to maintain accurate and up-to-date records
  • Valid driver’s license, current auto insurance, and reliable transportation
  • Pass required background checks with no evidence of fraud, waste, abuse and cleared to work by the Iowa Department of Health and Human Services.
  • Ability to operate a variety of office equipment to include but not limited to computers, printers, and scanners. Working knowledge of Microsoft Office Suite software.
  • Valid driver’s license and an acceptable driving record in accordance with risk management standards.
  • Ability to travel extensively throughout the community.
  • Physical ability to assist with lifting individuals when required.
  • Clear and effective oral and written communication skills.

Responsibilities

  • Work with support staff to ensure implementation of services as outlined in the Crossroads, Inc. Person Centered Support Plan (CRI PCSP).
  • Attend monthly staff meetings and provide on-site training as needed.
  • Coordinate development of CRI PCSPs and update documentation within required timelines to reflect changes in service needs.
  • Conduct monthly face-to-face meetings with individuals in their homes to monitor service effectiveness and satisfaction.
  • Provide coaching, mediation, and problem-solving support to individuals & Interdisciplinary Team members to resolve conflicts and strengthen relationships.
  • Collaborate with Management to ensure recommendations are implemented promptly.
  • Serve as the primary point of contact for stakeholders.
  • Coordinate and facilitate referrals with internal and external partners.
  • Train the Lead Direct Support Professionals (LDSPs) using established agency training tools within designated timelines.
  • Ensure staff compliance with training guidelines established by Management.
  • Schedule and conduct regular one-on-one meetings with LDSPs to provide guidance, set priorities, and address challenges.
  • Complete performance evaluations within required timelines and provide ongoing performance feedback.
  • Assist with hiring, disciplinary actions, and professional development for assigned caseload.
  • Assume or delegate necessary duties during LDSPs vacancies, including working direct care shifts if needed.
  • Review site schedules and budgets with LDSPs to ensure services remain within approved budgets.
  • Collaborate with the Finance Department to develop individual and site budgets aligned with CRI PCSP requirements.
  • Address and resolve financial and budgetary concerns for assigned caseloads.
  • Complete payroll documentation within designated timelines.
  • Review and approve LDSP timesheets in accordance with Finance Department policies.
  • Complete 16.25 hours of Relias training within the first 30 days of employment.
  • Participate in the on-call rotation.
  • Assist clients with personal and everyday tasks as needed.
  • Monitor and assist with client transportation.
  • Participates in team meetings and other organizational activities as required.
  • Establish and maintain positive working relationships internally and externally.
  • Communicate respectfully, openly, and professionally; share pertinent information in a timely manner.

  • Identify and remove barriers to services to ensure individuals receive appropriate support.
