About The Position

Bayberry Inc. provides supported living services to clients with developmental disabilities. The SLS Coordinator supervises the Personal Assistants I, Ill who are responsible to support clients in their own homes. SLS Coordinator responsibilities include but are not limited to the following:

Requirements

  • Applicant must: Be 18 years of age or older.
  • Have at least one year of experience working with persons with developmental disabilities.
  • Have independent and reliable transportation, along with a valid driver's license, insurance, and clear DMV record
  • Must obtain or provide all necessary clearances to begin work, including: Appropriate fingerprint clearance. TB test with negative results (or Doctor's exam for TB positive result)
  • Have ability to be reached by telephone reliably.
  • Be able to handle crisis situations effectively, and according to protocol.
  • Be able-to work independently.
  • Be able to follow-up on directives from supervisor.
  • Be dependable, arriving to work on time.
  • Be able to read, write; and comprehend English.
  • Be able to utilize the computer in order to communicate via e-mail, write client reports track client and staff data, monitor payroll and HR functions for employees providing direct care, etc.

Nice To Haves

  • Bachelor's Degree preferred.
  • Experience may be considered in lieu of a degree.

Responsibilities

  • Schedule Personal Assistants to provide support to clients in their own homes pursuant to the goals and objectives in the client's Individual Program Plan and purchase of service authorization from the Regional Center.
  • Supervise Personal Assistants who provide care and supervision to clients in their own homes and in the community, and monitor performance to ensure that the following responsibilities are carried out consistently and effectively per the client's IPP and ISP: Administer medications or assist with medication administration, as appropriate to the client. Assist client with Daily Activities Household chores Food shopping/meal preparation Persona/ care and hygiene Emergency preparedness Mobility assistance Provide transportation to appointments to include, medical, dental, counseling, planning team meetings, etc. Participate in/schedule social and recreational outings with client. Maintain awareness of client safety and follow risk management protocol. Promote and maintain a positive and supportive environment for the client.
  • Participate in the interview and hiring process for Bayberry, Inc.
  • Develop schedules for personal assistants, ensuring continuity and consistency for clients.
  • Ensure submission of timely and accurate time records for Personal Assistants via Baybeny, Inc. 's selected payroll system.
  • Responsible to participate in progressive discipline to address employee performance issues.
  • Participate in planning team process with client, sharing essential information to assist with planning team decisions.
  • Communicate any information that needs to be passed on to outside agencies i.e, to the regional center CPCs, and to other collaborative agencies as needed Ensure implementation of Individual Service Plan (ISP) and Individual Program Plan OPP).
  • Ensure tracking of data per client behavioral plans/IPP/lSP.
  • Participate in additional meetings regarding client as needed.
  • Provide direct client support along with Personal Assistants in cases of emergency and as needed to ensure the health and safety of clients.
  • Report special incidents to the regional center and licensing agency as soon as they occur (once reviewed by Program Director/ St-S Supervisor), pursuant to the Welfare and Institutions Code, Titles 17 and 22 Regulations.
  • Notify supervisor of any changes in client health or activity.
  • Notify supervisor of all medication changes per physician order.
  • Review of daily documentation activity in client file.
  • Complete quarterly/semi-annual/annual reports regarding client status to be submitted to supervisor for review.
  • Complete documentation to request continued regional center funding for purchase of service for each client.
  • Provide additional documentation as directed by supervisor.
  • Communicate professionally with supervisors, co-workers, clients, their families
  • Establish and maintain professional working relationships with clients, their families, all Bayberry staff.
  • Attend required Bayberry meetings.
  • Represent Bayberry in a professional manner at all times.
  • Perform other duties as assigned.
