Make a difference as a Support Coordinator As the Full-Time Support Coordinator at Owen Health Care, you will play a pivotal role in enhancing the lives of individuals with intellectual and developmental disabilities. Your responsibilities will include assessing and completing a Person-Centered Planning Tool and gathering necessary information with clients, their families, and the support team to identify goals, eligibility, outcomes, and required services. Utilizing the NJCAT, you will identify support needs and coordinate off-site team meetings at Residential and Day Treatment Programs to develop individualized support plans (NJISP). You will also work closely with families and mentors to establish desired outcomes and manage budgets effectively. By ensuring that service plans comprehensively address personal, medical, and employment issues, you'll be instrumental in connecting clients with qualified providers. Your diligent maintenance of individual records and proactive case management will ensure that services are delivered efficiently and in accordance with agency, state, and federal standards, paving the way for meaningful, positive change in the community.
