Support Broker (KRCC – Letcher County) Company/Program/Location: Kentucky River Community Care/Michelle P. Waiver program/Letcher County Employment Type: Regular Full-Time
Requirements
- Bachelor’s in Social Work or related and approved Human Services field
- Computer and documentation skills
- Background in DDID preferred.
Responsibilities
- Conduct monthly home visit with participants to assure health, safety, and welfare are being maintained.
- Provide training and assistance to help participants with Plan of Care and Budget.
- Complete assessments and reassessments for participants in a timely manner.
- Monitor service provision and assist in the development and completion of a person-centered plan of care
- Ensure PDS employee timesheets meet regulatory requirements.
