Presbyterian is seeking a Support Broker Provides support to Self-Directed Community Benefit members and assists the member or their legal representative in arranging for, directing and managing Self-Directed Community Benefit (SDCB) services and supports. Assists the member in developing, implementing and monitoring the SDCB care plan and budget. Collaborates with the member and Care Coordinator to ensure the member s health and safety needs are met. Requirements

  • Bachelor's degree in Social Work, psychology, human services, counseling, nursing, special education or a closely related field required.
  • 6 years of additional experience related to the delivery of social services to seniors and/or people living with disabilities can be substituted in lieu of degree.
  • Have one year of supervised experience working with seniors and/or people living with disabilities.
  • Must have a valid driver license, clean driving record, and ability to travel as needed throughout the state.
  • Proficiency in Microsoft Word, Excel and Outlook required.
  • Experience in analyzing trends based on decision support systems.
  • Business management skills to include, but not limited to, cost/benefit analysis, negotiation, and cost containment.
  • Knowledge of referral coordination to community & private/public resources.

Responsibilities

  • Educate members on how to use self-directed supports and services and provide information on program changes or updates
  • Contact the SDCB member telephonically at least monthly and in person at least quarterly for a routine follow-up.
  • Assist in managing budget expenditures and complete and submit SDCB care plan revisions
  • Assist with Employer Of Record (EOR) functions including recruiting, hiring and supervising SDCB providers, developing job descriptions and approving timesheets
  • Develop and closely monitor the SDCB care plan based on the SDCB budget amount determined by the annual Comprehensive Needs Assessment (C.N.A.)
  • Assist in the development of the backup plan and assess the adequacy as needed, but at least annually.
  • Assists with orientation and mentoring of new team members as appropriate.
  • Collaborate with the Care Coordinator, Employer of Record (if someone other than the member) and additional ICPT members as identified by the member.
  • Performs other functions as required.
