About The Position
Conduct monthly home visit with participants to assure health, safety, and welfare are being maintained. Provide training and assistance to help participants with Plan of Care and Budget. Complete assessments and reassessments for participants in a timely manner. Monitor service provision and assist in the development and completion of a person-centered plan of care Ensure PDS employee timesheets meet regulatory requirements.
Requirements
- Bachelor's in Social Work/Human Services
- Computer and Documentation skills required
- background in DDID preferred
Responsibilities
