Position Overview We are seeking experienced and compassionate Supervisory Registered Nurses (RN) to support our skilled home health team. This role is ideal for nurses who enjoy independent field work , clinical leadership , and delivering high-quality patient care in the home setting.
Requirements
- Graduate of an accredited BSN, ADN, or Nursing Diploma program
- Active DC RN license
- Minimum 1 year RN experience (acute care/Med-Surg preferred)
- Strong leadership and communication skills
- Comfortable using electronic health records (EHR)
Nice To Haves
- Home health experience strongly preferred
Responsibilities
- Perform comprehensive patient assessments
- Develop and update individualized care plans
- Provide skilled nursing services (wound care, infusions, medication management, catheter care, patient education)
- Supervise LPNs, CNAs, and direct care staff
- Monitor patient progress and communicate with physicians and leadership
- Complete Medicaid evaluations and re-certifications
- Ensure compliance with regulatory and agency standards
- Participate in discharge planning to support safe transitions of care
