Collaborates with health, behavioral health and social service providers and is responsible for the development and management of service plans for patients enrolled in a NYS-licensed Health Home who have need for the most complex, high utilizing and costly services and may be at high risk for under-utilizing services. Recruits, selects, orients, and trains employees and develops growth opportunities. Accomplishes staff job results by coaching, counseling, and disciplining employees. Plans, monitors, and appraises job results. Conducts training.

Requirements

  • Bachelor's degree in an appropriate human services field required
  • 4 years of experience in providing direct services to people with serious mental illness, intellectual/developmental disabilities, alcoholism/substance abuse, or experience effectively linking people with services that address social determinants of health or an equivalent combination of education and experience required
  • Or equivalent combination of education and experience
  • DL NUMBER - Driver License, Valid and in State with a satisfactory driving record and access to an automobile upon hire required

Responsibilities

  • Provides ongoing clinical and performance supervision for Health Home Care Managers assigned to team.
  • Monitors team and individual productivity, attendance, reviews and approves leave requests, adjusts caseloads to accommodate leaves of absences and addresses performance concerns in accordance with New York State Department of Health/Office of Mental Health.
  • Leads Health Homes organizational policies and procedures.
  • Provides consult and direction to the team and individual team members to ensure efficient delivery of timely, appropriate and effective services to the clients on caseload.
  • Provides performance feedback on a regular basis through individual and group meetings, annual performance evaluations, improvement plans and discipline as necessary.
  • Monitors caseload sizes, providing accurate enrollment data to the intake coordinator and coordinating/providing caseload coverage for members of team when out of the office.
  • Reviews and discusses care plans with care managers and patients, focusing on linking individuals to clinical and social services with system and community providers.
  • Monitors the use of costly Emergency and Inpatient services, supports patient in keeping all aftercare appointments and addressing barriers as needed.
  • Manages program specific supervisory responsibilities as assigned.
  • Oversees and completes incident reporting for care managers assigned to team as in compliance with regulatory oversight agencies.
  • Audits charts per prescribed auditing policies and on an as needed basis.
  • Ensures care manager recipients are receiving care management services at the appropriate level of care.
  • Participates as a member of the Health Home Care Manager recruitment and retention team.
  • Provides Health Home Care Management services to Health Home members as part of coverage and support for team when necessary.
  • In conjunction with leadership, plans operations in terms of short-term and long-term caseload coverage and participates in decision making that directly impacts program operations and policies.
  • Utilizes community and family resources to create sustainable support systems for patients, including within other programs and departments within the University of Rochester Medical Center.
  • Responsible for ensuring Health Home care management electronic documentation remains in compliance with regulatory oversight agencies.
  • Tracks and monitors to ensure the services connected to the client are delivered as planned and are effective.
  • Conducts in-services in collaboration with leadership on a variety of training topics, including specific Health Home Care Management policies and procedures, in addition to general care management practices, community resources, etc.
  • Fosters a supportive, diverse and inclusive work environment.
  • Other duties as assigned.
