About The Position

The vision of the MGH Charlestown is a responsive community health center that is open and welcoming to all residents of Charlestown and the surrounding communities and that strives to improve the care and outcomes for all patients while reducing the disparities in health care that exist for patients with any psychosocial and structural barriers. The Community Programs Supervisor is responsible for providing leadership, and supervision of the CHW/Navigation Team and the Medical Interpreters. This is a working supervisor position; therefore, in addition to supervising the case work of CHWs, the supervisor will also carry a case load. The Supervisor demonstrates and upholds the Charlestown Community Health Center's commitment to patients, staff, and community members to improve health and well-being through the equity lens. The individual is a dynamic, self-starter with the capacity to manage multiple concurrent tasks, a high focus on enhancing patient care, community engagement and quality improvement, and a commitment to team building practices. The Supervisor is responsible for providing leadership and assumes accountability for the department and the team and will actively engage in community initiatives that strengthen the bridge between the health center and Charlestown community.

Responsibilities

  • Supervise and manage the Navigation / Community Health Program staff in Charlestown, including Community Health Workers, and Medical Interpreters
  • Carry a CHW patient caseload
  • Work closely with the MGH Equity and Community Health Department Managers on continuous process improvement of various aspects of the programs and on professional development of staff.
  • Work closely with Health Center personnel to facilitate a culturally sensitive and welcoming environment for patients, staff, and providers
  • Ensure quality of staff work by connecting provider, patient, leadership, and community agency feedback and by closely monitoring program evaluation efforts.
  • Support staff development and enhancement of skills, so staff excels and avoid burnout.
  • Provide constructive shadowing and on-the-job training to support staff and to help staff identify solutions and plans of action.
  • Work with individual staff on conflict resolution and trouble shoot issues.
  • Provide individual coaching and reflective supervision which supports staff and allows staff to talk through challenges or concerns with patients and promotes their growth as professionals.
  • Ensure appropriate referrals and documentation in EPIC and other data bases as needed
  • Conduct one-on-one meetings with each staff member on a routine basis and hold performance reviews annually.
  • Lead internal initiatives to understand how to best meet the navigation needs of our patients. This includes but is not limited to heading the Patient Family Advisory Council and the Community Needs Committee.
  • Recruit and train new candidates for positions as needed.
  • Orient new staff to the MGH systems and closely monitor and guide their first three-month probationary period.
  • Active participation in health center management teams and activities
  • Lead program administration efforts, including strategic planning, managing programs, building and maintaining strong communication with diverse internal and external stakeholders and meeting all reporting requirements
  • Oversee time keeping for staff
  • Works alongside Administrative Director to participate in annual review process for staff as well as performance accountability
  • Oversees preparation and dissemination of educational and informational materials such as brochures, bulletin boards and visual aids in collaboration with departments to address various public health concerns and topics
  • Work closely with programs' clinical liaisons to ensure that program work is most effectively and most appropriately helping health center patients
  • Manage and assign referrals on EPIC Work queue to staff and review progress of cases in supervisions
  • Clarify the ongoing goals and objectives of each program
  • Meet all evaluation and reporting requirements including overseeing quantitative and qualitative data collection and analysis, including maintaining data bases and meeting benchmarks.
  • Work with evaluation staff to determine outcomes of and to measure the results of the programs.
  • Lead program improvement initiatives.
  • Work with staff to implement program initiatives and special projects, including, but not limited to: trainings, communication, education and community outreach strategies.
  • Collaborates heavily with MGH Equity and Community Health, following MGH model of community health work, leveraging existing work flows and best practices.
  • Develop, engage, and maintain strong relationships with key stakeholders, including health center staff and providers and community agencies.
  • Serves as the chair of assigned committees including but not limited to the MGH Charlestown Patient and Family Advisory Council and the Community Needs Committee.
