About The Position

The Supervisor of Community Health for LakeWood is responsible for ensuring quality and safe delivery of public health programs with respect to the Standards of Community Health Nursing Practice. The Supervisor of Community Health is responsible for coordinating provision of services that reflect departmental mission, vision, values and standards of care; plan, develop, implement and evaluate services, programs and activities; ensuring quality and safe delivery of services; appraising individual , family and population health needs and hazards, existing or potential; and providing health counseling to individuals, families and groups. The Supervisor of Community Health supervises other departmental staff: nurses, health aides and other personnel. The Supervisor of Community Health promotes the core functions of public health assessment, policy development and assurance.

Requirements

  • Graduate of an accredited school of professional nursing with current licensure in the State of Minnesota.
  • Certified in Public Health Nursing by the Minnesota Board of Nursing.
  • RN Licensure in the State of Minnesota

  • Researches, determines, and explains changes in nursing role to public; promoting caring, concerned attitude of LakeWood

  • Periods of stress due to illnesses, distraught relatives, scope of supervision

Responsibilities

  • Public Health department and RHC visiting nurse program.
  • Supervisor of Community Health responsibilities
  • High degree of responsibility to ensure quality and safe delivery of services.
  • Responsible, in part, for the financial control of personnel, supplies, and charges that must be made to patient’s account for service received.
  • Develop and implement written policies and procedures that reflect departmental philosophy and objectives.
  • Must assure that the unit meets standards set by the State Department of Health.
  • Promote harmonious relations within the department, between departments, and the community.
  • Supervision, development, and management of personnel.
  • Direct the public health nursing programs.
  • Risk management activities and quality assessment improvement programs in relation to public health programs.
  • Review contracts and acquires new contracts.
  • Works with other community resources.
  • Provides assistance/acts as a resource person to staff when making community referrals.
  • Shares expertise by mentoring, precepting, and/or teaching staff.
  • Maintain competency in computer software and overseeing office and electronic health records used by public health.
  • Provides leadership in problem identification and resolution and coordinates resolutions as needed.
  • Promotes and establishes an atmosphere of Quality Improvement throughout the agency with motivation, coaching and staff development and through evaluation development and/ or revision of policies.
  • Develops, reviews and approves Emergency Preparedness plans and roles within CHI LakeWood Public Health.
  • Completes ICS trainings (ICS 700, ICS 800, ICS 100, ICS 200, ICS 300, and ICS 400) and Medical Counter Measures training (in MN TRAIN) as capable through the Public Health Emergency Preparedness grant permits.
  • Identify community health needs and develop plans for improvement (CHIP) participate in the Community Health Services (CHS) Strategic Plan and the Quality Improvement Programming of the department and the North County CHS.
  • Attend and participate in meetings relevant to Public Health, provide educational trainings, as applicable, and promote positive community awareness regarding available services and health prevention/promotion topics.
  • Evaluate quality and productivity of services and grants through analysis of statistics, reports, time and cost studies in conjunction with staff, grant managers, and the North Country CHS.
  • Prepares and administers the budget for the department.
  • Makes sure timecards are accurate and signs off.
