FTE: 1.0, Shift: Day, Schedule: M-F Position Summary The Supervisor, Case Mgmt & Hospital Social Work is responsible for providing immediate oversight, support, and direction to assigned hospital-based care management work teams. The Supervisor acts as the primary designee in the absence of the Manager or Director, assuming additional responsibilities as delegated. This position is also responsible for serving as a liaison with clinical leadership and other stakeholders for decision-making, problem solving, and specific projects. The Supervisor serves as a role model and change agent, supporting the Manager or Director by supervising daily operations and providing timely feedback to support and maintain quality patient care in an efficient, productive and healthy work environment. In addition, the Supervisor of Care management may carry a modified patient case load, performing all the duties of a Hospital Social Worker, and ensuring compliance with CMS and other regulatory initiatives.

Requirements

Master’s degree from an accredited college or university in social work or related mental health discipline
LICSW (Licensed Independent Clinical Social Worker) within 2 years with continuing education courses required for licensure supervision per Washington State WAC.
Three (3) years of recent clinical experience in area of specialty
Experience with post-acute levels of care criteria and CMS discharge planning requirements
Previous experience and demonstrated skills in a supervisory or lead capacity
