As a Case Management Social Worker in our Emergency Department and Medical/Surgical units, you will be responsible for coordinating the care and services of patients through comprehensive psychosocial assessments, supportive counseling and linkage to community resources. This role involves intervention with individuals experiencing complex psychosocial or discharge planning challenges, provision of crisis intervention for patients and families, and facilitation of safe, effective transitions of care for high-risk patients. A caregiver in this role works days from 8:00 a.m. – 4:30 p.m. with every 4th weekend rotation and 1 holiday per year requirements.
