About The Position

The Supervising Case Manager will provide individual case management and informational services to senior citizens within Greenwich House’s Older Adults Network. This role will carry a case load of short- and long-term clients, while supervising other Case Managers and OAN staff. The Supervising Case Manager will be responsible for working alongside Senior Center Directors and other agency staff to identify and assist clients in need, with an end goal of enabling seniors to live with independence and dignity in the own homes. This individual will be required to interact with all levels of personnel, customers and clients of Greenwich House, as well as maintain cordial business relationships with the organization’s other programs.

Requirements

  • LMSW with SIFI
  • Experience working with the elderly and/or with at-risk populations
  • Fluency with benefits and entitlements
  • Must have interpersonal skills, ability to engage clients and families
  • Capacity to assess functional, cognitive, social status of clients
  • Must be able to work at other Greenwich House senior centers, off-site, and occasional home visits
  • Ability to identify and intervene in crisis situations
  • Ability to work collaboratively with interdisciplinary team to accomplish program goals and adapt to changing environment
  • Excellent written and verbal communication skills; proficient computer skills including Microsoft Word, Excel

Nice To Haves

  • Working knowledge of Spanish or Chinese, preferred. Russian or French, a plus

Responsibilities

  • Conduct client intake and assessments to determine their needs.
  • Provide information, case assistance, referral, supportive counseling, and follow up to assist with financial, social, health, and mental health needs.
  • Facilitate applications and renewals for benefits including SNAP, Medicaid, Medicare, Access-a Ride; assist with referral and connections to legal services, medical care, mental health services, home care, housing, and advocacy with public and private systems.
  • Timely documentation of services provided and client status in agency records and online data base (STARS), according to DFTA and Greenwich House guidelines.
  • Supervise, mentor and lead direct reports, as assigned by supervisor/program director.
  • Supervise MSW students.
  • Perform other tasks and special projects as assigned by direct supervisor.
