About The Position

The Case Manager will function as the service facilitator overseeing and ensuring the coordination and timely delivery of services to the student and family.

Requirements

  • BA / BS in Social Work, Masters preferred
  • Minimum one (1) year of Case Management experience working with children carrying developmental disabilities, medical involvement and/or challenging behaviors.
  • Required to have a Florida driver’s license and good driving record.
  • Excellent administrative, clerical and organizational skills.
  • Good business writing skills.
  • Highly organized and able to professionally interact with employees and families.
  • Computer skills with Google & Microsoft Office

Responsibilities

  • Meet with the S.T.E.P. Coordinator on a regular basis to discuss the program, the students, services concerns, problems, etc.
  • Function as the service facilitator overseeing and ensuring the coordination and timely delivery of services to the student and family.
  • Initiate and follow through with service referral process and coordinate scheduling any service or referrals.
  • Schedule and arrange for referrals as needed, comment sheet or other referral sources meeting program guidelines.
  • Conduct individual meetings with parents or students to offer guidance, suggestions and support referring families to parent support groups and/or professional counseling, if needed.
  • Make referrals to other service providers and coordinate delivery of ancillary services not provided through UCO.
  • Assist phone-ins or walk-ins wanting information about the program and services and refer to appropriate outside services and programs.
  • Act as a Sr. Transition Specialist as needed.
  • Actively participate in monthly S.T.E.P. staff meetings, teacher meetings, community meetings, etc.
  • Participate in all S.T.E.P. activities.
  • Include case notes of relevant information in the student’s records and advise appropriate staff of any additions or changes in information.
  • Review records to update and obtain information as needed. Report goals and any other documentation pertaining to students.
  • Report any cases of child abuse or neglect to DCF and coordinate reporting procedure and follow-up.
  • Submit monthly the accurate and complete data for each student who receives services.
  • Performs other duties as assigned.
  • Conduct classroom observations and assist teachers develop behavior programs and strategies for appropriate student working with the family to ensure carryover and consistency between the program and home environments.
  • Provide information to help families better understand their students’ handicap and special needs, help them set realistic expectations for themselves and their student and aid them in locating resources to help them in their parenting responsibilities.
  • Assess the needs of the family and assist them in gaining access to intervention services, advocacy services and other forms of assistance.
  • Consult with the staff and parents when concerns or problems arise, provide positive explanations and suggestions for the health and welfare of their student and follow up with additional contacts if needed.
  • Provide staff with in-service training in areas relating to social services, counseling, interpersonal relationships, communications, group interactions, behavior management, etc.
  • Participates in training and in-services when offered/required; Obtains re-certifications as necessary.
  • Pursues professional growth by making use of professional development opportunities that would enhance Ann Storck Center’s programs.
