At FBH we have a variety of client services, including outpatient, inpatient, community outreach, crisis services, partnerships with schools, law enforcement and more. Our evidence-based, culturally appropriate, trauma informed behavioral health services address the diverse needs of those we serve. The SUDP provides services to FPATH, FHARPS, and OCRP services. As a member of a multi-disciplinary team which includes Bachelor’s and Master’s level clinical staff and peers, the SUDP services to participants enrolled in FPATH, FHARPS, and OCRP services. The team provides outreach and engagement services to individuals who have been involved in the criminal justice system or who have been identified as high utilizers of hospital emergency services, resulting in their placement on the HCA referral list. FPATH works in close collaboration with OCRP, FHARPS, and Forensic Navigators to locate, engage and build rapport with identified individuals. FPATH will not duplicate services offered through health and behavioral health benefits provided under other programs but will leverage services otherwise available to enhance the services available to individuals on the referral list.
