About The Position

Our Community Based Substance Use Disorder Case Manager will provide case management services for persons in recovery. They will meet with PIRs on demand and via scheduled appointments to assist with linkage to external resources such as, but not limited to housing, recovery based supports such as 12-step meetings, medical benefits, PCP, specialty doctors, food banks. They will assist PIRs in obtaining valid, state identification. The case manager will uphold effective and timely communication with Supervisors and Counselors on PIRs needs and as resources or obtained and/or changed. They will assist Counselors as needed with referrals to a higher level of care and with transfers limited to ensuring ROIs are obtained and paperwork sent timely. This employee will have the ability to educate and encourage resistant PIRs to participate in therapy services and ability to multi-task and work in a fast-paced environment.

Requirements

  • Bachelor’s degree or higher in Psychology, Social work, Criminal Justice or related human services field required.
  • One (1-3) year of substance use disorder treatment required.
  • Have a credential through PCB (i.e.. CRS, CPS, CAAP) or be actively working toward certification (must obtain within two years of hire).
  • Valid driver’s license and ability to operate company owned vehicles required.
  • Hires after 7/1/2021 – employees must complete and maintain certification and/or license required by regulations and offered by Merakey within the required timeline. Failure to successfully complete and maintain license and/or certification and associated continuing education requirements will prevent you from being able to fulfill this role.
