About The Position

As a member of the leadership team the nurse practitioner serves as an expert clinician and role model in a subacute skilled nursing setting. Works collaboratively with physician counterparts to manage the health care needs for specified patient populations. Oversees and assists directly with pre-admission review and assessment of referred patients both in person at referral sites and through technology. Partners with Medical Director of Subacute and Director of Nursing to educate area and county medical facilities' personnel and medical staff on subacute criteria. Under the Palomar Health Medical Staff's purview and the Medical Director's sponsorship, acts as the point person on-site to deal with all subacute patients' clinical problems as within scope of practice defined by Medical Staff. Acts as primary source of contact for clinical questions and concerns of staff nurses and has the authority to evaluate and treat changes in patient conditions. Addresses patient and family concerns, and when beyond the scope or knowledge of the NP, calls the Medical Director or his surrogate. Provides primary source of clinical call to ensure continuity of care of subacute unit patients. As subject matter expert for subacute, provides: ongoing monthly education and just in time training to subacute staff to ensure the needs of existing and incoming patients are met; quarterly education for nursing staff in the form of a case study review; assists with oversight and monitoring of accuracy, quality and timely completion of required subacute clinical documentation which complies with regulatory standards of CDPH, CMS and Joint Commission; leadership of subacute IDT meetings and assists with the formulation of the initial and comprehensive plan of care; participation in meetings that support regulatory compliance and enhance clinical practice. Supports leadership and staff to meet and exceed patient experience goals facility-wide; and leadership of performance improvement projects which serve to enhance subacute processes and care and in conjunction with the Quality Assurance Performance Improvement Plan. Provides education and support to the patients and families regarding their condition and ongoing management of their care. Actively participates to meet and exceed staff engagement, facility quality metrics, and district goals. Functions as an active member of a multi-disciplinary team to coordinate efforts across the health system that maximize clinical and financial outcomes. Speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position. Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, Powerpoint, internet, e-mail). Windows computer skills including proficient use of keyboarding, use of mouse or keys for functions such as selecting items, use of drop down menus, scroll bars, opening folders, copying and similar operations required upon employment or within the 1st two weeks of employment to perform the essential functions of the job. Requirements

  • Minimum Education: As required by certification and/or licensure
  • Minimum Experience: 3 - 5 years clinical nursing
  • Required Certification: American Heart Association recognized BLS - Healthcare Provider
  • Required License: Current CA Nurse Practitioner License
  • Current CA RN License
  • Speak and read English at a level that is sufficient to satisfactorily perform the essential functions of the position.
  • Knowledge of standard office equipment (i.e., calculator, fax, photocopier) and personal computer and computer software skills (i.e., MS Windows, Excel, Access, Word, Powerpoint, internet, e-mail).
