About The Position

Responsible for the management of the Certified Primary Stroke Center. The Stroke Coordinator is the lead facilitator for the hospital departments, physicians, external agencies, nursing staff and allied healthcare professionals. The Stroke Coordinator ensures the care of the stroke patient throughout the continuum and works with the Medical Director of the Stroke program, collaborating with Stroke team members and nursing to ensure comprehensive medical record documentation to reflect clinical treatment, decisions and diagnoses for inpatients. The Stroke Coordinator manages all stroke data services and systems within Marin General Hospital. The Stroke Coordinator is responsible for the collection, abstraction, and clinical interpretation of data for patients admitted with TIA, ischemic or hemorrhagic stroke diagnosis at Marin General Hospital. This position currently also includes responsibility for data input, analysis, and presentations, for Endovascular Services

Requirements

  • Graduation from an accredited school of nursing with current licensure in the State of California as a Registered Nurse required.
  • Significant clinical experience in the field of cardiovascular / neurovascularservices is required, including a minimum of two years recent acute care experience, the ability to interpret data from the clinical record.
  • Excellent communications skills and knowledge of clinical terminology unique to Neurovascular and Endovascular services.
  • Knowledge of compliance and confidentiality is required.
  • The role requires the ability to work independently, set priorities, and identify and complete projects.

  • Knowledge of clinical paths or guidelines for endovascular and neurologic diagnoses to include: TIA, ischemic, hemorrhagic and cryptogenic strokes.
  • Ability to communicate well verbally and in writing.
  • Ability to work independently, set priorities, and identify and complete projects.
  • Ability to stay organized, work effectively with intricate and detailed subject matter, and use good deductive reasoning.
  • Ability to identify and set priorities consistent with departmental and organizational goals.
  • Ability to work well in a fast-paced, demanding environment.
  • Proficiency (or ability to become proficient) with computer operating systems, including database, spreadsheet, and word processing programs.
  • Ability to design and run queries and produce standardized reports.

Nice To Haves

  • A bachelor’s degree is preferred.
  • Overall experience with computer operating systems and proficiency with Microsoft productivity software (including Accessä, Excelä and Wordä) is preferred.

Responsibilities

  • Maintains compliance with Joint Commission Standards and Core Measures for Stroke in order to maintain Primary Stroke Center Certification Status
  • Performs concurrent and retrospective data abstraction from patient records, interfaced data, and daily rounds.
  • Inputs data into the appropriate program (network software program, , Excel, MIDAS, develops and maintains hospital based databases for additional stroke and endovascular related data collection
  • Ensures compliance with mandated reporting programs and national data registries.
  • Works with physicians, physician assistants, nurse practitioners, nursing and IT support staff to develop and evaluate data collection systems for multiple databases.
  • Develops and implements appropriate systems within daily workflow to ensure data completeness and accuracy.
  • Maintains primary responsibility for the following data registries: Get With The Guidelines ..AHA/ASA National Registry MIDAS Stroke Core Measures Vascular Quality Initiative…Society Of Vascular Surgeons Registry
  • Runs queries for departmental outcome studies, preparing standard monthly and annual reports for vascular and neurology services committees.
  • Interprets and presents reports to Neuroscience Committee, Neurology Division and Endovascular Division and Performance Improvement Committee.
  • Represents Marin General Hospital and coordinates with Marin County EMS program for all combined stroke data projects.
  • Assesses, obtains and validates stroke and/or endovascular complications and outcome data.
  • Achieves proficiency using clinical database programs.
  • Assesses and reports any unusual occurrences regarding potential risk management or quality assurance issues verbally or in writing as soon as possible to Director or Quality Management Specialist.
  • Abides by all rules of confidentiality of medical information as guided by state and federal regulations and hospital and departmental policy.
  • Acts as primary contact for Information Technology (IT) support staff.
  • Works with MGH staff to ensure that the database software program and interfaces function properly.
  • Works with Quality Management and Medical Records to maintain accurate patient databases for coding and reporting.
  • Works with Education Department assisting in staff education (Healthstream)
  • Attends staff meetings as able / as requested to assist in education of staff regarding various stroke core measures and policies
  • Communicates with nursing and ancillary staff regarding stroke patient census, compliance with core measures and documentation.
  • Assists Medical Director of Stroke Program with Neurology/Stroke Rounds
  • Reports stroke data monthly to various departments via email ; provide feedback to Marin County EMS
  • Concurrent EMR review to ensure compliance with all Stroke Core Measures; communicate any potential or real non-compliance to physician involved, Chairman of Hospitalists and / Stroke Program Medical Director.
  • Stroke related policy/ procedure and order development
  • Other duties as assigned.
