About The Position

The Per Diem Street Medicine Physician provides reasonable and prudent medical/surgical care. They will practice appropriate management, using a harm reduction trauma informed approach with sound clinical judgment based on best practice standards and evidenced based medicine. Provides general medical care, patient education and treatment to patients of CS-HHC. Promotes and maintains health by providing services under the supervision of the Clinical Director of Homeless Care and the Chief of Medicine.

Requirements

Degree from an accredited Medical School.
Documented completion of Residency training from an accredited institution.
CPR Certification is required.
Board Certification or Board Eligibility.
Connecticut Licensure.
Admitting privileges at Yale-New Haven Hospital, HSR, and/or Griffin Hospital as indicated.

Responsibilities

Maintains confidentiality in regard to patients, staff and program matters at all times.
Consults with other staff or specialists to make appropriate referrals for acute, chronic and preventive health care, as may be indicated.
Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary.
Educates patients on preventive health care and chronic disease management.
Treats patients and staff respectfully, courteously, appropriately.
Maintains a safe and clean working environment by complying with procedures, rules and regulations.
Protects clients and employees by adhering to infection-control policies and protocols.
Completes the charting of notes, forms, and coding in a timely and efficient manner. Enters appropriate legible notations, documents the encounters appropriately.
