About The Position

The Step-Down Navigator is responsible for meeting all eligible patients to discuss the benefits of stepdown services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to eligibility, into Midas for trending report purposes. The navigator will have a thorough understanding of the step-down process and will coordinate between departments. The Step-Down Navigator will be responsible for completing all necessary paperwork for admission with patients.

Requirements

  • Graduate of an accredited college in a social work program or related field.
  • A minimum of three years of experience in the care of mental health patients preferred.
  • Must possess CPR (AED) certification and successful completion of CPI training including appropriate use of seclusion and restraint prior to first shift.
  • Knowledge of external review organization, payor sources, financial planning and contracting (ie. Managed care, Medicare and Medicaid, Tricare) preferred.
  • Must have required 20 hours of continuing education, CPR, First Aid, and CPI as provide by the facility within 90 days of employment.
