About The Position

Built in partnership with the Hospital for Special Surgery (HSS)—the world leader in MSK health trusted by elite athletes—we’re creating a new standard of coordinated, expert-led care that gets people moving better and faster. We partner with PCPs, Orthopedic specialists, and health plans to deliver high-value, patient-centered MSK care. Our value-based care model includes patient engagement, virtual physical therapy, and care navigation to high-value imaging and specialty partners. We are looking for a Physical Therapist who will be responsible for delivering virtual musculoskeletal evaluation, triage, and care planning. This individual will assess patients, determine the appropriate level of care, initiate and progress conservative management when indicated, identify red and yellow flags, and collaborate closely with Care Coordinators and medical partners to ensure appropriate escalation when needed He or she will report to the VP of Care Delivery.

Requirements

• Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program.
• Active, unrestricted physical therapy license in the state(s) where care will be delivered.
• Minimum of 3 years of clinical experience, with preference for 5 years.
• Strong clinical reasoning skills and a patient-first approach to care.
• Comfort practicing in a virtual environment, including remote evaluations and video-based patient education.

Nice To Haves

• APTA specialist certification (e.g., OCS, SCS).
• Experience delivering care in virtual or telehealth-based models.
• Experience working in interdisciplinary or integrated care teams alongside medical providers.
• Spanish language proficiency.

Responsibilities

• Conduct virtual musculoskeletal triage and follow-up visits to assess symptoms, functional limitations, and clinical risk.
• Apply strong clinical reasoning to determine appropriate clinical recommendations within a virtual care model.
• Deliver evidence-based virtual physical therapy interventions focused on functional improvement and symptom management.
• Develop individualized care plans aligned with patient goals, functional needs, and long-term outcomes.
• Identify clinical needs requiring care coordination and route tasks to Care Coordinators
• Partner with Advanced Practice Providers (APPs) and other clinicians when medical escalation or specialty involvement is indicated.
• Communicate and coordinate with patients, care coordinators, APPs, and external referral sources to support continuity across the episode of care.
• Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory, payer, and organizational requirements.
• Adhere to established clinical protocols, escalation pathways, and quality standards within a value-based care environment.
