About The Position
Assists individual nutritional assessments and development of care plans for program participants. Performs related duties supporting program operations.
Requirements
- Bachelor's Degree required, or equivalent combination of education and related experience.
- 0-1 years of relevant experience, required.
Responsibilities
- Assists nutritional assessment and tailors food package for program participants.
- Follows nutrition care protocols.
- Assists the implementation of client specific high risk nutrition care plans.
- Assists in monitoring client status and modifies care plan, as needed.
- Refers clients to community services, as needed.
- Prepares individual and group education classes as per policy.
- Assists in monitoring staff working in kitchens, cafeterias, and dining rooms to ensure maintenance of premises and equipment in good sanitary condition.
- Utilizes effective lines of communication regarding nutrition information within the program and community.
- Documents actions in department computer system; maintains records and writes reports.
- Performs related duties as required.
