About The Position

The SSTF Housing Case Manager is a part of a comprehensive service team that provides evidence based integrated health services, including mental health, substance use, and primary healthcare. This position is responsible for providing assessment, case plan development, housing placement, skill building support, and linkage to community resources. In addition, this individual will also be in charge of monitoring housing compliance and services necessary to assist individuals and families in obtaining and sustaining long-term housing stability through a collaborative process, including full engagement and integration into the patient care team, providing input into care plans, and alerting medical staff of psycho-social issues complicating health conditions, if and when a client chooses to enroll in integrated health services. Note: This contractual position is a grant funded, contingent position, ending 5/2026 with the potential for future funding opportunities. Selected candidate will receive a $5,000.00 signing bonus.

Requirements

  • Bachelor’s Degree in Social Work or related human service field
  • Staff should possess a minimum of 1-3 years’ experience working with people experiencing homelessness
  • Trained and proficient in housing case management, mental health first aid, motivational interviewing, and trauma-informed care
  • Ability to be credentialed as a Qualified Mental Health Professional (QMHP)
  • Possession of a valid driver’s license and access to a private vehicle for day-to-day job performance
  • Knowledge of available community resources, services, and partner organizations
  • Demonstrated excellent customer service skills and professionalism toward internal and external customers
  • Ability to maintain cultural awareness and sensitivity to differing backgrounds
  • Ability to communicate effectively both orally and in writing with employees, managers and the general public
  • Demonstrated ability manage time, organize and prioritize work assignments
  • Proficient in use the use of all MS Office applications, Office 365 and familiarity with other web-based solutions
  • Flexibility in days/hours available for scheduled work, including evenings and weekends
