About The Position

As our Sr. Patient Logistics Coordinator, you will be responsible for providing and coordinating care to patients, providers, care team members, and organizational leaders, utilizing critical thinking and problem-solving skills. Every day, you will collaborate with other members of the Clinical Command Center team, delivering effective and appropriate care within a safe and confidential environment. You will be accountable for advancing personal and professional development that supports care innovation. Possessing excellent communication skills, you will maintain an overall understanding of continuum of care needs and patient experience outcomes. You will effectively and efficiently identify needs of the patient and care team members, guiding resources to the right functional role or performing appropriate roles/workflows to progress patients through the care continuum. To be successful in this role, you will possess extensive experience in patient care coordination, logistics, or a command center environment within healthcare. You will demonstrate exceptional critical thinking, problem-solving, and communication skills, capable of navigating complex patient journeys and collaborating effectively with diverse stakeholders. Inputs accurate clinical documentation. Onboards new staff members and provide support to transfer center staff. Collaborate with site ECMO coordinators across the division to ensure timely access to care for the most crucial population. Coordinate services provided to external patterns such as radiology, laboratory testing, endo and services not offered at the referring external partner. Facilitate emergent transfer requests such as Trauma, Strokes & STEMIs. Assist in activation of emergent teams across the division for incoming critically ill/time sensetive patients.

Requirements

  • High School Graduate General Studies or equivalent combination ofexperience/education., upon hire or High School GED General Studies or equivalent combination ofexperience/education., upon hire
  • At least one (1) year as a staffing coordinator or admitting or patientplacement in a hospital or health carerelated setting
  • Knowledgeablein medical terminology and previousbackground in a hospital or health caresetting
