About The Position

The Sr. Case Manager Vista Haven provides trauma-informed, individualized case management services according to ORR and other regulatory requirements. The Sr. CM is responsible for covering case management cases and for covering on assigned weekends to ensure the 7-day case management ORR policy. The Sr.CM is responsible for mentoring the case management team and exemplifying a strong working relationship with the ORR and stakeholders. The Sr.CM acts as a secondary reviewer for all cases to ensure that youth in care are reunited with their sponsor in a timely manner. Sr.CM is to mentor, cover, process, and abide by the ORR UC Policy and ORR UC Map. The Sr.CM works closely with the Federal Field Specialist, Case Coordinator, and Contracted Field Specialist to vet all identified sponsors for the resident in a timely manner for the purposes of family reunification.

Requirements

  • Master's degree in behavioral sciences, human services, or social services fields or a bachelor’s degree in social work, psychology, or a related field is required.
  • Four years of experience working with youth with mental health-related issues or residential and youth experience.

Nice To Haves

  • Two years’ experience in case management is preferred.
  • Experience with ORR Standards preferred.
  • Bilingual in any language preferred.

Responsibilities

  • Acts as a mentor to the Case Management Team and abides by the 7-Day Case Management service delivery standards.
  • Captures the daily performance of the CMs and CMAs and informs the Lead Case Manage
  • Coordinates with the LCM to cover caseloads when a case manager takes sick or vacation time.
  • Coordinates with the LCM to cover for an absent CM to meet with the resident at least once a week or as needed to establish and assess progress towards timely unification with the sponso
  • Assist residents in obtaining personal documents such as identification, legal documentation, and other relevant information to complete the case and prove a relationship with the sponsors.
  • Meet with the assigned caseload at least once weekly or as needed to establish and assess progress toward short-term goals, such as extracurricular activities, mental health resources, transitional support, and ongoing long-term planning and discharge planning.
  • Responsible for documenting interventions per ORR standards and meeting requirements to update the UC Portal, case note all engagements regarding all entities spoken to in the case, update the case review (weekly), complete assessments, update the ISPs, and when applicable.
  • Weekly, communicates with the YDC team members to assess the case’s progress and retrieve resident updates on their day-to-day function.
  • Participates in family sessions and or family therapy sessions to track accountability for tasks and activities assigned to complete the FRA and sponsor vetting.
  • Coordinates meetings and makes recommendations with the resident's Child Advocate, FFS, consulates, and Legal Service Provide
  • Communicate all case-related updates to the Program Director (PD), Federal Field Specialist (FFS), Case Coordinator (CC), and Contracted Field Specialist (CFS) on an as-needed basis and seek support from these entities to move forward with case approval
  • Complete Significant Incident Reports (SIRs) to document disclosed incidents impacting the resident's health and safety.
  • Submit the Notice of Placements (NOP’s), Evidentiary Records, and Transfer Checklists for transfer youth (step up, step down, LTFC).
  • Update caseload trackers and submit data points on time to ensure case organization.
  • Completes specialized training related to the case management role assigned by ORR and CCL.
  • Ensure all documentation is filed in the resident's main paper file to meet all compliance standards. Additionally, ensure all items are uploaded and logged on the UC Portal.
