About The Position

Hospice of the East Bay (HEB) provides compassionate, community-based home care for terminally ill patients, with support for families that extends for a full year after the loss of a loved one. Care is delivered by a dedicated interdisciplinary team, ensuring holistic support for both patients and their families. HEB’s services are available regardless of the patient’s ability to pay, thanks to donations and insurance billing that help sustain the program. POSITION SUMMARY: Reporting to a clinical manager, this position is responsible for providing spiritual counseling services to patients and families when requested, either directly or indirectly through referrals to local clergy. The Spiritual Care Counselor does not impose upon the patient/family the dictates of any value or belief system, but shall support them in exploring and utilizing the spiritual resources available to them.

Requirements

  • Demonstrates the skills, knowledge and abilities necessary to be successful in the job as outlined in the position requirements.
  • Provides on-call telephone support services to patients as required by rotation on weekends.
  • Must have excellent interpersonal and communication skills.
  • Understanding of spiritual needs of families in crisis.
  • Ability to assess issues and provide spiritual care as needed.
  • Ability to work effectively with families and groups.
  • Ability to speak to the public effectively.
  • Ability to provide culturally sensitive support to people from diverse ethnic, cultural and socioeconomic backgrounds.
  • Must work well under pressure meeting multiple and sometimes competing deadlines.
  • Must at all times demonstrate cooperative behaviors with colleagues and supervisors.
  • Must demonstrate clinical competency as observed during supervisory visit.
  • At least one (1) year of clinical experience as chaplain or pastoral care required.
  • Completion of at least one (1) Clinical Pastoral Education (CPE) units required.
  • Current CPR certification.
  • If a member of the clergy, must be in good standing with theological or spiritual community with which he/she is affiliated.

Nice To Haves

  • Prior experience in providing spiritual care to the terminally ill preferred.
  • Knowledge of impact of grief and loss on family structure desirable.

Responsibilities

  • Completes a spiritual assessment on all consenting patients/families.
  • Develops a plan of care for patient/family with the interdisciplinary team.
  • Provides spiritual counseling to assist patients/families to integrate the dying experience into their lives and to find meaning and purpose.
  • Documents in a timely manner all patient/family contacts, communications with interdisciplinary team members and includes this documentation in the patient’s clinical record.
  • Facilitates contact with local pastoral care community at patient and family request.
  • Conducts memorial/funeral services when requested.
  • Participates in educational programs and workshops for HEB staff and the community at large.
  • Attends and participates as a proactive and supportive member of the interdisciplinary team.
  • Adheres to state and federal regulations, as well as accreditation standards.
  • Responsible for following the Agency’s process for effective “hand off” communication.
  • Provides support as needed to HEB employees and volunteers.
