As a Spiritual Care Counselor, you will offer presence, reflection, and compassionate support to patients and families navigating serious illness and end of life. This role is well suited for clinicians and chaplains trained in spiritual care, including those with Clinical Pastoral Education, who value inclusive, non denominational support grounded in listening and connection. Working alongside an interdisciplinary team, you will help create space for meaning, comfort, and dignity for individuals of all beliefs and life experiences. Overview The Spiritual Care Counselor provides emotional and spiritual support to patients and families of all faiths, beliefs, and cultural backgrounds. Working in accordance with the hospice philosophy and Medicare Conditions of Participation, this role supports the diverse spiritual needs of individuals through non-denominational, inclusive care. The Counselor works closely with the interdisciplinary team to promote comfort, dignity, and connection at the end of life.

Requirements

  • Bachelor’s degree in psychology, counseling, religious studies, social work, or related human services field required; master’s degree preferred.
  • Experience providing emotional or spiritual support in hospice, hospital, behavioral health, or community settings.
  • Demonstrated ability to support individuals and families of all belief systems, including secular or non-religious populations.
  • Ability to address grief, life transitions, and existential concerns with compassion and respect.
  • Comfort working in homes, facilities, and diverse care environments.
  • Understanding of hospice philosophy, goals of care, and Medicare Conditions of Participation related to spiritual care.
  • Strong interpersonal, communication, and documentation skills.
  • Valid driver’s license and reliable transportation to make patient visits as required.
  • Excellent interpersonal, documentation, and communication skills.
  • Ability to work independently while engaging collaboratively with interdisciplinary team members.
  • Proficiency in using EMR systems and virtual communication tools.

Responsibilities

  • Provide individualized spiritual and emotional support to patients and families throughout the end-of-life journey.
  • Conduct spiritual assessments and develop patient-centered care plans reflecting diverse cultural, spiritual, or secular needs.
  • Offer presence, listening, life review, and legacy support without promoting any specific belief system.
  • Coordinate care with Chaplains, social workers, or community spiritual leaders as appropriate.
  • Participate in interdisciplinary group (IDG) meetings and contribute to holistic care planning.
  • Assist with grief support and bereavement follow-up, including participation in memorial services or community grief events.
  • Document all interactions and care planning updates in the EMR system according to policy and regulatory standards.
  • Collaborate with Volunteer Coordinator and other team members to align support across the care continuum.
  • Provide education to team members on topics related to spiritual distress, cultural humility, and psychosocial-spiritual care.
  • Maintain professional boundaries, confidentiality, and a spirit of compassion and inclusivity in all interactions.
  • Remain current on best practices in spiritual care and end-of-life emotional support.
  • Perform other duties as assigned.
