Who You Are as a Spiritual Care Coordinator You are someone who leads with compassion, empathy, and presence. You understand the importance of meeting patients and families where they are—emotionally, spiritually, and personally. You are comfortable supporting individuals from a variety of faith traditions, beliefs, and backgrounds, and you know how to offer comfort during difficult and often sacred moments. You are a strong communicator, a calm presence, and someone who truly values holistic patient care.

Requirements

  • Experience in spiritual care, pastoral care, ministry, chaplaincy, hospice, home health, or a related setting
  • A compassionate and patient-centered approach
  • The ability to provide support to patients and families from diverse backgrounds and belief systems
  • Strong communication and documentation skills
  • The ability to work independently while collaborating with an interdisciplinary team
  • A valid driver’s license and reliable transportation for travel within the service area

Responsibilities

  • Provide spiritual support and counseling to patients, families, and caregivers
  • Assess spiritual needs and help incorporate them into the patient’s overall plan of care
  • Offer comfort, guidance, and emotional support during illness, decline, grief, and end-of-life care
  • Collaborate with nurses, social workers, aides, and other members of the interdisciplinary team
  • Participate in care planning and team meetings
  • Support patients and families in accordance with their personal beliefs, values, and preferences
  • Document visits, interventions, and patient/family needs in a timely manner
  • Serve as a spiritual support resource for both patients and staff as needed
