About The Position

Requirements

• Licensed SLP
• Physical exam within 6 months prior to first patient contact
• Current TB test (within 12 months) & CPR certification
• Valid driver’s license & proof of automobile insurance
• Ability to attend 1–2 day orientation at our office

Nice To Haves

• Minimum 1 year of SLP experience preferred
• OASIS and WellSky (Kinnser) experience strongly preferred

Responsibilities

• Evaluate patients, develop individualized treatment plans, and administer therapy in the home
• Supervise COTAs and HHAs according to the treatment plan
• Chart patient progress, maintain accurate medical records, and submit notes timely
• Collaborate with families, caregivers, and interdisciplinary team
• Review and recommend adaptive equipment or home modifications as needed
• Maintain patient confidentiality per HIPAA regulations
• Coordinate total patient care and submit referrals for paramedical services
